A Renault Megane was heading out of North Devon when two surfboards flew off the roof racks and hit a convertible Triumph Spitfire driving in the opposite direction.
The Triumph was said to have swerved to avoid the boards and collided with a Peugeot.
Both passenger and driver of the Triumph had to be prized from the car by firefighters. They are currently in hospital, with the driver having "sustained very serious injuries considered life changing".
The Renault driver was taken to the Royal Devon and Exeter Hospital and had a broken wrist and damaged vertebra in his neck, said the Mid Devon Gazette.
